The aim of this project is to assist the dialog between physicians and patients, when time comes to take decision in Intensive Care Unit (ICU). This brochure should be easily printed by the physician at his own desk and aims to explain clearly some medical procedures to the patient. Therefor, it has be be convenient to produce and to understand. To do so, visual indicators have been added to guide reading and memorising. Once filled up, this document can be added to the medical records of the patient.
